With the demand for botulinum toxin injections, including Botox, Dysport and Xeomin, as strong as ever – surpassing 4 million procedures performed according to stats released March 8, 2016 by the American Society for Aesthetic Plastic Surgery (ASAPS) -MilfordMD Cosmetic Dermatology & Laser Center cosmetic dermatologist Richard Buckley, M.D., says the popularity of botulinum toxin injections doesn’t tell the whole story. Trending is the approach to botulinum toxin injections, according to Buckley. The frozen look that left many patients without expression is old school, according to the industry trendsetter. In fact, the goal, according to the Milford, Penn., cosmetic dermatologist, is: “Not too tight. Not too little. Just right.
Patients who want to lose the deep furrows or wrinkles between their brows, don’t want to lose their freedom of expression, according to Buckley, medical director of MilfordMD Cosmetic Dermatology Surgery and Laser Center.
“Frozen isn’t natural,” Buckley says. “What many patients don’t realize is that they can become frozen and expressionless over time, or from having botulinum toxin injected incorrectly (too much or in the wrong place) in one procedure. We always strive to retain a patient’s freedom of expression, while attacking the facial furrows, wrinkles and lines that make a person look older and angrier. It’s a very fine balance and it takes experience.”
Physician injectors need to be intimate with facial anatomy—able to identify, among other things, a person’s musculature. It is important to note that book anatomy knowledge is only part of successful injecting. Everyone’s faces (and underlying muscles) work a little differently, according to Buckley.
“The only way to achieve optimal results with Botox or other botulinum toxins is to observe the patient go through a series of facial expressions. I note which muscles are strong and which are weaker and base my injecting pattern and amount on those observations,” he says.
Patients are also different in how much botulinum toxin they need to achieve the desired result.
“Some people require a lot of material to achieve any amount of relaxation, and some people require very little,” Dr. Buckley says.
Taking all the individual’s nuances into consideration, Dr. Buckley approaches botulinum toxin injections, such as Xeomin, to the glabellar lines (between the eyebrows) and elsewhere with a reasonable, but conservative dose of product. The true test is in a week to 10 days, when Dr. Buckley sees all patients for their follow up appointments.
“That’s when we look critically at how the patient has responded to the botulinum toxin. Most of the time, it’s just perfect and patients love it. In other cases, we might need a little more relaxing of the muscles to achieve greater control of facial lines and crevasses. I note our findings in the patient’s chart, so that we hone the perfect dose to achieve the patient’s objectives going forward,” he says.
Just like with any cosmetic procedure, according to Dr. Buckley, the goal is to create a naturally pleasing effect—one that makes a person look more relaxed, happier, more youthful and, most of all, pleased when they look at themselves in a mirror.
