A lot has changed in the nearly three decades since the cosmetic practice first opened its doors to patients, according to cosmetic surgeon and MilfordMD founder and Medical Director, Dr. Richard Buckley.

In the early years, Dr. Buckley and his staff concentrated on medical and cosmetic skin issues—from diagnosing and removing skin cancers to removing lumps, bumps and other unsightly skin conditions. The foundations of good skin health haven’t changed, but the approaches to maintaining healthy, youthful skin have. The approaches (treatments) have gotten better. One example: Dr. Buckley and his staff have always recommended sun block, to reduce the evidence of skin cancer and slow skin aging. Today, products to protect the skin from the sun’s unwanted effects have improved, to include products that block both UVB and UVA.
Devices and technologies used to treat skin aging have grown in leaps and bounds. “One of our goals throughout the years has been to research what’s new and focus on making available the most effective novel treatment options and devices to our patients,” Dr. Buckley says. But while the technological bells and whistles are important to patients. It is something quite different that keeps them coming back, the cosmetic surgeon says.
The key to success in a practice that focuses on treating cosmetic concerns, according to Dr. Buckley, is to understand the full scope of what is troubling the person and to try to help them with a sensitive, respectful comprehensive plan. “This often means helping patients understand the scope of what might help them in their lives that may have nothing to do with medicine. For example, a patient may have just lost someone dear to them and it may not be the best time to add the possible stress of a bigger procedure, such as a facelift, even though the patient thinks that could be the key to happiness,” he says.
There are so many facial rejuvenating options today—from invasive to non-invasive—that Dr. Buckley says there is no one-size-fits-all solution. He can, and does, offer options that fit into the patient’s life and current situation, while addressing the cosmetic concerns at hand. “Everyone wants to be happy and we want to help patients set a reasonable course towards that goal in any way we can,” he says.
“Twenty-nine years ago, Botox did not exist. Restylane and the other hyaluronic fillers did not exist. None of the lasers that we use today existed. There was no Thermage, no Ultherapy, no Venus Freeze,” Dr. Buckley says. “The only filler we had in 1987 was bovine collagen, known as Zyderm and Zyplast. We can use today’s fillers on the spot, so that patients have results that night for a party or other occasion. Back then, we had to do a skin test before injecting bovine collagen. The skin test needed to be performed and read a month later, before patients could receive filler. To top it off, the early fillers didn’t last much beyond three to four months, compared to fillers today, which can last a year or more.”
