A new study published in a scientific journal finds that dermatologists are the leading innovators in 14 of 18 categories of non-invasive and minimally invasive cosmetic procedures. Dr. Richard Buckley, a cosmetic surgeon who specializes in dermatological conditions and practices at MilfordMD Cosmetic Dermatology Surgery & Laser Center, in Milford, Penn., says he’s not surprised by the findings but consumers likely will be.

A study published in the recent issue of Dermatologic Surgery, the official publication of the American Society for Dermatologic Surgery, reveals that dermatologists have led in the development and research of 14 of 18 of today’s most commonly performed noninvasive and minimally-invasive cosmetic procedures. “This study is important because many people don’t understand differences among medical and non-medical specialties, and why certain medical specialties (dermatology, in this case) are better trained than others to perform things like Botox injections, hyaluronic acid filler injections, laser resurfacing, tumescent liposuction and much more,” says Richard Buckley, M.D., a cosmetic surgeon who specializes in dermatologic conditions and medical director of MilfordMD Cosmetic Dermatology Surgery & Laser Center, in Milford, Penn.

Researchers looked at published studies for 18 procedure categories, including a few cosmetic surgeries, such as nose reshaping, and found that dermatologists had developed or done more research than plastic surgeons; ear, nose and throat (ENT) specialists; OB/gyn; and many other physician and non-physician specialties in these areas:
• botulinum toxin injections, which include Botox, Dysport and Xeomin, for skin aging
• laser hair removal
• chemical peels
• cryolipolysis, which is known as fat freezing or CoolSculpting
• calcium hydroxylapatite fillers, such as Radiesse, for wrinkles
• high-intensity focused ultrasound, such as Ultherapy
• hyaluronic acid fillers for wrinkles, including Restylane and Juvéderm
• laser rejuvenation
• laser resurfacing
• laser tattoo removal
• poly-l lactic acid fillers for wrinkles, such as Sculptra
• tumescent liposuction
Plastic surgery was the clear winner in innovation and research for liposuction, sclerotherapy (for vein treatment), hair transplantation, as well as male breast reduction and nose reshaping. “If you go back in history, plastic surgeons were primarily hospital-based doctors involved in the reconstruction of injuries, for example, during the Civil War. The focus of plastic surgery, historically, was not about the cosmetic aspects but rather about restoring function. And there is still a major component of that in plastic surgery today, but, over time, plastic surgeons began to promote an improved cosmetic experience for patients,” Dr. Buckley says.
The roots of the specialty of dermatology, on the other hand, came from diagnosing and treating diseases of the skin, according to Dr. Buckley. “Dermatologists were not used to doing large surgeries. If they did any surgery at all, it would be a small biopsy or removing a skin cancer. So, those two groups are professionals who have moved in the direction that makes sense for the skill set that they originally trained in. Surgeons do the bulk of cosmetic surgery and dermatologists are most experienced in nonsurgical and minimally invasive cosmetic procedures,” he says.
