With New Botox Statistics Just Out by the ASAPS, Dr. Richard Buckley Defines Botox Trend: Not Too Tight. Not Too Little. Just Right.

With the demand for botulinum toxin injections, including Botox, Dysport and Xeomin, as strong as ever – surpassing 4 million procedures performed according to stats released March 8, 2016 by the American Society for Aesthetic Plastic Surgery (ASAPS) – MilfordMD Cosmetic Dermatology & Laser Center cosmetic dermatologist Richard Buckley, M.D., says the popularity of botulinum toxin injections doesn’t tell the whole story. Trending is the approach to botulinum toxin injections, according to Buckley. The frozen look that left many patients without expression is old school, according to the industry trendsetter. In fact, the goal, according to the Milford, Penn., cosmetic dermatologist, is: “Not too tight. Not too little. Just right.”

Patients who want to lose the deep furrows or wrinkles between their brows, don’t want to lose their freedom of expression, according to Buckley, medical director of MilfordMD Cosmetic Dermatology Surgery & Laser Center.

“Frozen isn’t natural,” Buckley says. “What many patients don’t realize is that they can become frozen and expressionless over time, or from having botulinum toxin injected incorrectly (too much or in the wrong place) in one procedure. We always strive to retain a patient’s freedom of expression, while attacking the facial furrows, wrinkles and lines that make a person look older and angrier. It’s a very fine balance and it takes experience.”

Physician injectors need to be intimate with facial anatomy—able to identify, among other things, a person’s musculature. It is important to note that book anatomy knowledge is only part of successful injecting. Everyone’s faces (and underlying muscles) work a little differently, according to Buckley.

“The only way to achieve optimal results with Botox or other botulinum toxins is to observe the patient go through a series of facial expressions. I note which muscles are strong and which are weaker and base my injecting pattern and amount on those observations,” he says.

Patients are also different in how much botulinum toxin they need to achieve the desired result. “Some people require a lot of material to achieve any amount of relaxation, and some people require very little,” Dr. Buckley says.

Taking all the individual’s nuances into consideration, Dr. Buckley approaches botulinum toxin injections, such as Xeomin, to the glabellar lines (between the eyebrows) and elsewhere with a reasonable, but conservative dose of product. The true test is in a week to 10 days, when Dr. Buckley sees all patients for their follow up appointments.

“That’s when we look critically at how the patient has responded to the botulinum toxin. Most of the time, it’s just perfect and patients love it. In other cases, we might need a little more relaxing of the muscles to achieve greater control of facial lines and crevasses. I note our findings in the patient’s chart, so that we hone the perfect dose to achieve the patient’s objectives going forward,” he says.

Just like with any cosmetic procedure, according to Dr. Buckley, the goal is to create a naturally pleasing effect—one that makes a person look more relaxed, happier, more youthful and, most of all, pleased when they look at themselves in a mirror.

MilfordMD Reports Nonsurgical Skin Tightening Success with Ultherapy

FDA-cleared, non-invasive Ultherapy treatments are now offered at MilfordMD Cosmetic Dermatology Surgery & Laser Center. Take advantage of our Spring Ultherapy specials!
Are you ready for tighter, better-fitting skin? Ultherapy is a non-invasive rejuvenation treatment for the face, neck and cleavage that stimulates your own collagen to naturally lift from the inside.

In an age of selfies and high-definition images, nonsurgical skin tightening is in high demand, and there’s one new technology that stands out for its safety and results, says MilfordMD Cosmetic Dermatology & Laser Center cosmetic dermatologist Richard E. Buckley, M.D. “We’ve recently added Ultherapy and patients are raving about it,” Buckley says.

Ultherapy stands alone as the only FDA-cleared device that noninvasively lifts skin to elevate the eyebrows, tighten the skin on the neck and under the chin. Ultherapy is also FDA-cleared to diminish lines and wrinkles in the often hard-to-treat décolletage area, says Buckley, medical director of MilfordMD Cosmetic Dermatology Surgery and Laser Center.

“Nonsurgical skin tightening is an important option for people who want to look naturally young and vibrant from the brow to the chest area,” Buckley says. “This particular device, Ultherapy, safely uses tried-and-true ultrasound technology, which allows me to precisely treat areas that need subtle tightening and smoothing. Without drugs or surgery, the treatment stimulates the natural formation of collagen and elastin to gradually smooth wrinkles and tighten skin.”

Nonsurgical skin tightening rose from the 9th to the 7th most popular procedure among nonsurgical cosmetic options. It secured the 5th spot for most popular procedures among men, according to statistics released March 8, 2016 by the American Society for Aesthetic Plastic Surgery.

Does nonsurgical skin tightening take the place of a facelift? No, according to Buckley. But it does offer an option for those who might not be ready for a facelift, as well as others who want to look refreshed without having to endure surgery and the downtime associated with it.

“Highly focused ultrasound (HIFU) is a successful concept, which has been under development for decades,” Buckley says. “Ultherapy’s use of ultrasound imaging device allows me to visualize precisely what I need to treat in order to tighten and lift tissue. It’s the only device out there at this time approved to not only tighten, but also lift.”

Patients see more definition and less sagging in the treated areas with just one treatment, which takes about 30 minutes and can be done in the comfort of the office. Results take shape during the three to twelve months post-procedure. And, while some may experience slight temporary bruising after the procedure, there is no downtime or recovery, which makes Ultherapy very convenient for patients according to Dr. Buckley.

Patients should know, however, the treatment can be somewhat uncomfortable, if doctors don’t use certain techniques, Buckley says.

“I had the procedure, myself, with no pain therapy to better understand what patients go through,” Buckley says. “Doctors typically recommend that patients take a couple of strong anti-inflammatories before Ultherapy, but that’s not always enough to make patients comfortable during the procedure. What we do is use very light tumescent anesthesia creating something called a ring block, which numbs the periphery of where I’m working. That approach results in an extremely comfortable procedure, with less post-procedure bruising. It’s a win-win.”

The décolletage treatment option, which Ultherapy’s maker Ulthera announced in July 2014, was the first noninvasive procedure specifically indicated by the FDA to improve wrinkles and lines on the chest area.

“The décolletage is a very visible area on a woman or man; yet, there was little we could do to rejuvenate it before Ultherapy became an option,” Buckley says. “Now we can seamlessly tighten and lift skin to improve the face, neck and chest, which not only improves those selfie images but also boosts self-confidence.”
