Neck Contouring With Kybella is a Hot Trend in Cosmetic Surgery, But Dr. Richard Buckley Warns Consumers to Choose Carefully and Not Get Sucked Into the Hype!

Milford, PA March 30, 2016 – Neck contouring is big news because of two recently approved approaches, Kybella and CoolSculpting’s CoolMini. But while the hype is around what’s new, what’s tried and true, for example liposuction with SmartLipo, remains an even better neck contouring option for people bothered by aging or genetically thicker necks, according to Dr. Richard Buckley, medical director at the cosmetic surgery practice MilfordMD, in Milford, Penn.

“I think Kybella and CoolSculpting can be options for some men and women concerned about neck fat without skin laxity, but it’s important to warn consumers that while Kybella is getting all the press because it’s new, it’s not the only option for neck contouring.”

The FDA approved Kybella, which is deoxycholic acid, last year for treatment of moderate-to-severe submental, or neck, fat in adults. What makes it different and attractive to many is that it’s not your typical surgical device, rather, it’s an injectable drug. People who are weary of surgery are considering Kybella because it may permanently and nonsurgically contour the neck. The active drug in Kybella, deoxycholic acid, does this by destroying fat cells.

But there are things that consumers should know, which might not appear in big letters in advertisements for the procedure. The drug, when injected incorrectly, can destroy other types of cells, such as skin cells, according to the FDA’s press on the approval. It is worth noting that a non-FDA approved product containing deoxycholic acid plus phosphatidyl choline, Lipodissolve, was taken off the market by the FDA due to a plethora of undesirable side effects. Lipodissolve had only a 35% “worth it” rating on realself.com.

And while injections of Kybella are not “surgery,” the substance does cause a lot of swelling, bruising, pain, numbness, redness and hard areas where patients have been treated. The very word injectable often is perceived as non-serious or safe. That’s not always the case. It’s important to go to someone with experience in the procedure. In rare cases, Kybella can cause serious side effects. One such side effect is nerve injury in the jaw, which can result in trouble swallowing, facial weakness or an uneven smile. Additionally, many treatment sessions are required and it is expensive.

The FDA also expanded clearance for CoolSculpting’s CoolMini applicator to treat smaller pockets of fat, including those around the neck area. Also noninvasive, CoolSculpting works via a process called cryolipolysis, which basically is using cold to cause fat cell death (apoptosis). This, too, is not free of the possibility of serious complications, including rare instances of something called paradoxical adipose hyperplasia, which is the accumulation of fat, instead of a reduction, in the treated area.

“The message to consumers is that there are potential complications from even noninvasive or nonsurgical approaches. The safest approach is to go to a physician with a good track record in neck contour procedures,” Dr. Buckley says. “That physician shouldn’t be pushing a particular option, but rather tell the patient about all the options.”

In the case of neck contouring, SmartLipo or PrecisionTx laser plus microcannular liposuction with while-awake tumescent anesthesia can remove the fat in the neck in a lunchtime procedure.

“It takes very little time. I can do one in 20 minutes. The neck looks immediately smaller. Whatever fat you sucked out is immediately gone. People get beautiful results. And it’s less expensive at our practice than Kybella,” Dr. Buckley says. “The problem is it’s a tried-and-true approach, without the marketing sizzle of some of the newer options.”

Despite the popularity of noninvasive cosmetic options, liposuction remains today’s most popular surgical procedure, and it’s the only surgical procedure in the top 10 cosmetic procedures, overall, according to American Society for Aesthetic Plastic Surgery’s March 2016 statistics.

Device manufacturers are fueling demand for noninvasive fat removal with heavy marketing efforts aimed at promising slimmer bodies with no surgery. But Dr. Richard Buckley, who performs both laser liposuction with SmartLipo, and noninvasive body contouring, says that comparing liposuction to noninvasive fat removal is like comparing apples to sesame seeds.

Dr. Buckley, a cosmetic surgeon and medical director of MilfordMD Cosmetic Dermatology Surgery and Laser Center, says liposuction has come a long way and remains the gold standard option for permanent fat removal.

“Historically, surgeons would use liposuction as a surgical procedure in the hospital, using big thick tools and incisions that would need to be closed. Patients back then endured a lot of downtime and recovery from general anesthesia,” Dr. Buckley says. “That procedure would do nothing to tighten the skin because the devices were only meant to remove fat. But, then, along came lasers.”

Researchers have long known that when physicians apply specific laser light to the outside of the skin, the skin gets tighter. They asked the question if lasers would have the same effect from under the skin and found they did.

“What that means to the patient is not only fat removal but also a more contoured and tightened result,” Dr. Buckley says. “With our SmartLipo Triplex device, we have a side firing laser, so part of the laser beam under the skin can be directed upward. In essence, the laser light is tightening the underside of the skin. With the SmartLipo Triplex laser, it’s no longer just sucking out fat; rather, it’s about contouring the body and tightening the skin.”

Today’s SmartLipo technology allows experienced cosmetic surgeons to restore esthetic beauty by working on where they know a woman’s curves should be. “It’s sculpting. It’s contouring. It’s artwork,” Dr. Buckley says.

To create true works of art, surgeons don’t insert suction cannulas or laser fibers under the skin and spend all their time on the fat pocket. Instead, they treat entire areas, spending more or less time depending on an area’s fat thickness.

Men and women who have laser liposuction with SmartLipo might want a more defined, sporty look, according to Dr. Buckley. “For that, we use state-of-the-art high-definition body sculpting. The high-definition result gives patients defined abdominal muscles, for example,” he says.

With the demand for botulinum toxin injections, including Botox, Dysport and Xeomin, as strong as ever – surpassing 4 million procedures performed according to stats released March 8, 2016 by the American Society for Aesthetic Plastic Surgery (ASAPS) – MilfordMD Cosmetic Dermatology & Laser Center cosmetic dermatologist Richard Buckley, M.D., says the popularity of botulinum toxin injections doesn’t tell the whole story. Trending is the approach to botulinum toxin injections, according to Buckley. The frozen look that left many patients without expression is old school, according to the industry trendsetter. In fact, the goal, according to the Milford, Penn., cosmetic dermatologist, is: “Not too tight. Not too little. Just right.”

Patients who want to lose the deep furrows or wrinkles between their brows, don’t want to lose their freedom of expression, according to Buckley, medical director of MilfordMD Cosmetic Dermatology Surgery & Laser Center.

“Frozen isn’t natural,” Buckley says. “What many patients don’t realize is that they can become frozen and expressionless over time, or from having botulinum toxin injected incorrectly (too much or in the wrong place) in one procedure. We always strive to retain a patient’s freedom of expression, while attacking the facial furrows, wrinkles and lines that make a person look older and angrier. It’s a very fine balance and it takes experience.”

Physician injectors need to be intimate with facial anatomy—able to identify, among other things, a person’s musculature. It is important to note that book anatomy knowledge is only part of successful injecting. Everyone’s faces (and underlying muscles) work a little differently, according to Buckley.

“The only way to achieve optimal results with Botox or other botulinum toxins is to observe the patient go through a series of facial expressions. I note which muscles are strong and which are weaker and base my injecting pattern and amount on those observations,” he says.

Patients are also different in how much botulinum toxin they need to achieve the desired result. “Some people require a lot of material to achieve any amount of relaxation, and some people require very little,” Dr. Buckley says.

Taking all the individual’s nuances into consideration, Dr. Buckley approaches botulinum toxin injections, such as Xeomin, to the glabellar lines (between the eyebrows) and elsewhere with a reasonable, but conservative dose of product. The true test is in a week to 10 days, when Dr. Buckley sees all patients for their follow up appointments.

“That’s when we look critically at how the patient has responded to the botulinum toxin. Most of the time, it’s just perfect and patients love it. In other cases, we might need a little more relaxing of the muscles to achieve greater control of facial lines and crevasses. I note our findings in the patient’s chart, so that we hone the perfect dose to achieve the patient’s objectives going forward,” he says.

Just like with any cosmetic procedure, according to Dr. Buckley, the goal is to create a naturally pleasing effect—one that makes a person look more relaxed, happier, more youthful and, most of all, pleased when they look at themselves in a mirror.

Patients see more definition and less sagging in the treated areas with just one treatment, which takes about 30 minutes and can be done in the comfort of the office. Results take shape during the three to twelve months post-procedure. The décolletage treatment option, which Ultherapy’s maker Ulthera announced in July 2014, was the first noninvasive procedure specifically indicated by the FDA to improve wrinkles and lines on the chest area.
